Long term conditions are physical health conditions that require ongoing management over a period of years.

Long Term Conditions include:

– Asthma

– Atrial Fibrillation

– Chronic Obstructive Pulmonary Disease (COPD)

– Chronic Kidney Disease (CKD)

– Dementia

– Diabetes

– Heart Failure

– Hypertension

There are other conditions that are classed as a Long Term Condition and your practice will invite you for reviews to help manage your condition. These may be referred to as a Long Term Condition Review or a Chronic Disease Management Review.

South Blaby & Lutterworth Primary Care Network is committed to supporting practices with their Long Term Condition reviews and is focussing on some of the areas to ensure patients are either reviewed or identified so that we can look at preventative measures. The main areas of focus for the PCN in 22/23 are:

– Atrial Fibrillation – Ensuring the correct prescribing of anticoagulant medication and the review of this medication

– Hypertension – Reviewing patients that have had a previous recording of a high BP and asking for 7 day home BP readings to review the potential need for an ongoing review.
